Ascend Rehab Services, Inc and Jeena presents:    

Summer Camp 2010 based in Milpitas, CA

Purpose: To organize summer camps for children filled with fun, music, laughter and play while learning and growing each day under the careful guidance of OT/PT and SLP. We want the extraordinary to shine in each child and for them to feel empowered.  We want them to be exposed to multiple activities and challenges so that learning becomes play and play blossoms into learning. 

Time: 2 groups from 9 am to 12 pm and 2 groups from 1 pm to 4 pm (M-F).  Each group will have 30 hours of therapeutic intervention directly with OT/PT and SLP over 2 weeks.  Individual sessions will be also offered in the afternoon as needed for PT/SLP and OT for 1 hour each (50 minutes 1:1 with child and 10 minutes consult with parent/family)

Age groups: 2-3 YRS, 3-5 yrs, 6-8 yrs, 9-12 yrs, 13 yrs onwards, Physical handicap class, individual sessions in the afternoon, other groups might be added based on interest and enrollment. Every child will be placed in an appropriate class as per his/her skills, needs, goals, age etc.

Ratio:  2 children and 1 adult (maximum 4 children in each group to provide personalized attention hence 1 OT and 1 assistant. PT/ST consult will be done 1x/week for both the weeks of summer camp)

Dates:  1^st session begins: 19^th July to 30^th July, 2010 (2 weeks or 10 days of summer camp) and 2^nd session from 2^nd August to 13^th August, 2010 (2 weeks or 10 days of summer camp).

Program Design:                                                                                                                                             

Begin with Circle time/warm ups and stretches: 15 min

Sensory motor groups: 30 minutes (obstacle courses, equipments etc to be used)

Table Top or fine motor activities or Handwriting Without tears: 15-20 minutes (dependent on age, skill and goals)

Oral motor activities/Feeding facilitation: 15-20 minutes.

Snack Time: 20-30 minutes (during which self feeding, utensils and feeding will be facilitated. Family to provide snack)

Self help activities/Attention span facilitation or eye hand coordinating activities: 20 minutes

New activity of the day: 30 minutes: This will be different each day e.g. Mondays: can be Therapeutic Listening, Tuesdays: Yoga for children, Wednesdays: Brain Gym and perceptual motor activities, Thursdays: Massage, sensory diet with handouts given, Fridays: Core muscle strengthening, movements and music. PT and ST consult to be done 1x/week.

Closing Circle: 5-15 minutes: Goodbyes and parent update about the session& provide handouts as needed etc.

Evaluation/consult: All families will be required to fill initial forms about their children.

Last 2 day of the group: a 20-30 individual consult will be done with each family/parent and a written note will be given with home exercise program to be followed after the summer camp. Any and all handouts will be given as needed and as requested.

PT consult will focus on gross motor facilitation, core muscle strengthening, body awareness, balance and coordination. SLP consult will focus on auditory reception/perception, functional communication and conversational skills. Language Intervention to focus on developing vocabulary, basic concepts, and communicating needs. OT will be the main therapist who will focus on    finemotor, self help, sensory integration, oral motor, feeding, eye hand coordination, therapeutic listening, movements, handwriting etc over the summer camp.

Insurance/super bills: This will be provided to each family at the end of the summer camp. Ascend is in network with United Health Care and BlueShield and Blue Cross, Tri-care etc.

Cost of 2 weeks summer program to be held at Jeena site: $875 for 30 hours of therapeutic intervention. (2 weeks of summer camp with direct OT/PT/SLP lead by Ascend Rehab Services, Inc with Jeena in Milpitas, CA). Additionally 30 minutes of individual consult with each parent/family with OT. Initial phone or in person consult to discuss concerns, goals, strengths, challenges etc prior to the camp. At the end of the camp, individual child’s home exercise program and follow through will be given in writing. Notes and suggestions from PT/SLP will also be given at the end of the camp to be followed at home. This program is not funded by any regional centers.

Cost of Individual sessions: only during summer camp: $65/session for individual session held at Jeena in Milpitas.
